By checking box above and submitting this form, I certify that I am the owner or owner’s agent, and authorize Monarch Air, it’s employees, subcontractors or consultants to perform the inspection, maintenance or repairs described above to include the provision and use of necessary materials required to accomplish the described work scope. I understand that customer supplied parts and materials must have documentation per the current regulatory guidance, and will be subject to a handling fee of 15% of current market value. I further authorize Monarch Air to operate the aircraft, engine(s), or any part therein described for the purpose of testing and/or inspection. I agree that I have filled out a Monarch Air Work Authorization Form and accept the terms and conditions. I agree to hold Monarch Air harmless from any and all claims arising from the loss of or damage to the aircraft, engine(s), appliance(s) or articles of any nature contained within the aircraft due to circumstances beyond Monarch Air’s control. I understand that payment for all work performed is due in full upon completion.
